A malfunction and double human error are believed to have caused the train collision
Initial findings from the investigation into the train accident in Urda, Spain, suggest a combination of technical failure and human error. The automatic signaling system, LZB, malfunctioned, leading to a manual control situation where human mistakes occurred. The Iryo train was stationary due to the signaling failure and had warned the control center ten minutes before the collision. However, the traffic controller failed to stop the Renfe train approaching in the same direction, which was expected protocol. The Renfe train was likely traveling at high speeds, contributing to the severity of the crash, which resulted in at least 64 fatalities. This incident is more severe than a previous accident in January, which had 46 deaths, indicating operational failures rather than infrastructure issues.
