The chain of errors that caused the train accident in Toledo

A serious train accident in Toledo revealed a series of technological and human errors. The initial trigger was the failure of the LZB signaling system, disrupting automatic communication between trains and control centers. Despite protocols requiring a maximum speed of 40 km/h under such conditions, the control center mistakenly instructed the AVE driver to resume normal operations, allowing speeds of up to 200 km/h. Meanwhile, the Iryo train reported being stopped, but operators failed to connect the two incidents. This lack of oversight and coordination lasted for 17 critical minutes until the fatal collision occurred.
