The Railway Accident Investigation Commission requested in 2024 measures to "mitigate" human errors after an incident similar to that of Urda

In 2024, the Railway Accident Investigation Commission (CIAF) urged for technical measures to reduce human error risks in railway operations. A report highlighted that visual checks can lead to incorrect decisions by railway personnel, especially in critical situations. This warning followed an incident at Cuenca station, which bore similarities to a tragic accident in Urda, resulting in at least 68 fatalities. The report emphasized that the only effective barrier against collision risks relied on visual confirmation by the circulation manager, which is prone to human error. The Urda accident was linked to a software failure in the LZB traffic management system, leading to a signal violation by a train. The CIAF noted a systemic issue, with 15 similar incidents reported since 2023, indicating a recurring problem rather than isolated events.
